We help U.S. adults age 50 and older request information about final expense insurance options that may be available in their state. We are not a government agency and we are not affiliated with Medicare, the Social Security Administration, or any other public program.
2. Information we collect
When you submit the request form on this site, we collect the information you provide, which typically includes:
- Your first and last name
- Your age range and state of residence
- The coverage amount you are interested in
- Your phone number and email address
- The date and time of your request and your consent selections
We may also automatically collect limited technical information such as your IP address, browser type, device type, referring page, and pages viewed on this site. This information is used to operate and secure the website.
We do not knowingly collect information from anyone under 18, and this service is intended only for adults age 50 and older.

By submitting the form, you consent to share your information with one or more state-licensed insurance agents, agencies, or insurance carriers who can review final expense options that may be available in your state. We may also share information with service providers who help us operate the site (for example, hosting, form processing, and communications providers), under contracts that require them to protect the information.
We do not sell your information to unrelated third parties for their own marketing outside of the licensed insurance professionals described above.
5. Phone calls, text messages, and email (TCPA)
By submitting your information and checking the consent box, you expressly consent to be contacted by American Family Benefit and one or more state-licensed insurance agents or agencies at the telephone number and email address you provided, including by automatic telephone dialing systems, artificial or prerecorded voice messages, and SMS text messages, even if the number is on a state or federal Do Not Call list. Consent is not a condition of purchasing any product or service. Message and data rates may apply. Message frequency varies. You may opt out at any time by replying STOP to any text message, asking to be removed during a call, or contacting us at the address below.
6. Your choices and rights
You can request that we access, correct, or delete the personal information we hold about you, and you can opt out of further marketing communications at any time. Residents of certain states (including California, Colorado, Connecticut, Virginia, and others) may have additional rights under state privacy laws, including the right to know what personal information we have collected and to whom it has been disclosed.
